	Submission of this form constitutes agreement to the following terms and conditions: Dogs are required to be on a leash and have current rabies vaccinations. Owners are fully responsible for keep control of their dogs. I agree to indemnify and hold harmless ODH and its sponsors and volunteers from all liability associated from the walk. I grant permission to use any photographs, video or other records of the event for any legitimate purpose.
